Terminal Learning Objective
Action: Evacuate the casualty.
Conditions: Given a classroom and outdoor training area, an Aid bag
with medical supplies, a litter and any modes of transportation, after
treating a casualty in a tactical area of operations, you must transport
the casualty away from the combat zone.
Standards: Evacuate the casualty using any mode of transportation at
hand while monitoring the casualty to determine any further treatment
needed.
Risk: Low.

Tactical Evacuation Care
Evacuation is the last phase of the three phases of TCCC.
After all life threatening injuries have been found and treated and the
patient is as stable as possible, prepare the casualty to be moved to
– A safer place locally
– A safer place further away from the combat zone
– Or a higher echelon of care such as a medical treatment facility or
a hospital

Moving the Casualty Locally
Move the casualty out of the immediate area first.
• Removing the wounded will avoid further injury to the casualty
Consider the appropriate extraction method.
– If possible enemy in the area or still under contact, use a drag
method to keep a low silhouette.
– If no enemy present at the moment, use a buddy carry to expedite
movement of casualty.
Take casualty to the last known safe location and ensure to continue
engaging the enemy if necessary.

Evacuation
Consider if casualty needs to be transported right away.
– Will evacuation of casualty put you or others in immediate danger?
– Is he/she going to die right now? Is it that serious of an injury?
– Is it a safe time to evacuate the casualty?
– What is the safest possible route?
– What personnel are going/escorting the casualty? Do I need them?
– What equipment could they take with them or bring back?

Choose who will escort the casualty. Ensure personnel has the
capabilities to keep casualty alive en route if needed.
– Aid bag / Supplies
– Knowledgeable. Knows what to look for.
– Can you afford to give him/her up as an escort because of other
duties? (Handles radios, experienced in map reading, leadership
capabilities, etc.)

Be aware of surroundings during travel. If in a ground vehicle,
anticipate an ambush down the road.
Constantly monitor and reassure the casualty
Reassess any treatment performed
– Is bleeding control still effective?
– Is he/she conscious or unconscious?
– Is his/her airway still good?

Breathing - If casualty is having trouble breathing, allow them to sit
up. If not, reassess chest, back, neck and armpits to determine the
problem. Ensure no constricting straps, clothing or interventions are
emplaced around chest and abdomen.

As soon as you arrive, ensure you give a proper patient hand-off. It is
important that you brief the next person
– Who the casualty is. (Name, unit, outfit, etc.)
– What happened, at what time, and what injuries occurred
– What treatment the casualty/casualties have received
– Any documentation made on patient care

After Evacuation
Once you have given all necessary information to the next caregiver,
you are done escorting the casualty.
Consider any refitting you or others may need before returning to
duty.
– Replace any equipment used or lost for the next casualties.
– Clean out casevac vehicle of any personal items belonging to
previous casualty, garbage or unneeded items that take up too
much space to make room for more casualties.
– Refuel if possible.
Return to your unit.
